Odisha Farmer Registration Form Download For Direct Subsidy Transfer on Fertilizer & Seeds

Government of Odisha started the process of direct fertilizer subsidy transfer to farmers’ bank account. So, farmers of Odisha need to Register their name and other details such as, land record, bank account details on FSCW's official website www.portal.foododisha.in.

To get direct subsidy on Bank accounts of farmers for the Kharip season, farmers need to fill the following form and then submit the filled form at their nearest Krusi Samabaya Samiti or Primary Agriculture Credit Societies (PACS) or at Large Area Multipurpose Cooperative Societies (LAMPS).

PACS or LAMPS will add / update farmers details on   portal.foododisha.in website.

Important details list need to fill: District name, Block/ULB name, Village, GP, Ward name, PIN Code, Farmer's Name, Gender, Category (GEN, SC, ST), date of birth, Age, Father/Husband name, Voted ID number, Aadhaar Number, Mobile number, no. of family members, family members details, Bank account details, such as: Name of Account holder, Account Number, Bank Name, Branch Name, IFS Code. Land Record details, etc.

With filled registration form, submit your Bank passbook front page xerox copy, Aadhaar & Voter ID xerox copy too.
