Board Of Secondary Education, Odisha going to conduct Online Form Fill-up process for Matric (10th) HSC Annual / Madhyama Examination 2016 from 28/10/2015 to 12/11/2015. Read following posts to know how to fill the form correctly without any mistake. For details instruction, please download the Printable Documents attached at the end of this post. Click on "APPLY ONLINE" button to fill the form.  

a. The online filling up of forms will be done on clicking the following button from 28/10/2015 to 12/11/2015 midnight.

b. All the work related to online filling up of forms shall be completed within the stipulated period including submission and deposit of fee at the bank.

c. The Head of the Institutions are the authority to Log In for filling up of application forms of their Class-X students (Regular & Ex Regular) for appearing at the Annual H S C Examination- 2016.

d. User ID will be the SCHOOL CODE and Password will be school@123 for all. The Head of the Institution has to immediately change the password which will be created here as follows:
Password must contain at least six characters with at least one number (0-9), at least one lowercase letter (a-z) and at least one uppercase letter (A-Z).

e. After Log In, the "School Profile" section will be displayed on the screen and different sections such as: School Profile, Regular Registration, Ex- Regular Registration, Registered Students List for Edit, Payment, Declaration and Descriptive Rolls will be displayed at the top of the screen one after other.

a. By clicking on to this section, certain data of students who have been enrolled during 2014 when they were studying in Class- IX will be displayed on the screen.

b. Here you have to select the candidates who will finally fill up their application forms for appearing at the Annual H S C Examination -2016.

c. Before selecting those candidates, their Internal Assessment Grades in WE, AE & HE have to be filled up first and then choose script opted for other subjects only in case the candidate‟s first language is other than FLO.

d. There after the box at the extreme right hand corner against the candidate has to be clicked for selecting the candidate.

e. Select only the candidates who have come forward to fill up the forms.

f. Filling up of forms can be done more than once but not of the same candidate again and again. Hence do not wait for all the candidates to arrive and fall into the traffic rush.

g. Take all possible steps to avoid traffic jam and complete the work as early as practicable, that is much ahead of the last date notified.

h. Click on “SAVE & NEXT” button and go to the section “REGISTRATION EX-REGULAR”.

a. Completing the registration for Regular and Ex-Regular candidates, Click on to this Section to view all the registered students list which can be EDITED if there exist any wrong in data already entered.

b. The Head of the Institutions will be given one more chance for correcting the  data already entered. If this chance is ignored, he/she shall be held responsible for all repercussions which will creep in, in due course.

c. It should be noted that without correction at this stage, the candidate may not appear the exam (for example in case of wrong third language subject), may not receive the correct pass certificate, may not clear the exam in case of compartmental category and many more. Hence Head of the Institutions shall be utmost careful in submitting this section, failing which they will invite Disciplinary Proceedings against them.

d. In this section there will be two options opened, one is “VIEW” and the other is “EDIT”.

e. The Head of the Institution are advised to VIEW the data of the candidate first and then click on to option EDIT if correction is required.

f. The correction for all category of candidates can be effected on the following data only:
Candidates Name, Mother’s Name, Father’s Name, Date of Birth & Language subjects (for regular candidates only). In case photograph is wrongly uploaded, correct photo can be re-uploaded.

g. The correction shall be in conformity with the official documents i.e Admission Register & Cancelled T.C of the Institution. In no case entire data shall be replaced. If it is detected at any stage that Head of the Institution has unlawfully replaced the data, he/she shall be liable for criminal prosecution.

h. Since the Head of the Institutions are getting ample scope to correct the data uploaded by them during enrolment, the hard copy of the D.R. i.e. the Pre-List suppose to be given to them for effecting correction in previous years shall not be issued to them this year. Hence the Head of the Institutions are once again advised to effect the correction in the data of their registered students in this section.

i. In case of Edit, an amount of Rs 100/- will be charged per candidate, which will be automatically reflected in the Payment section.

j. After completion of Edit, Click on “Submit & Go to Next” button to go to the section „Payment‟ and then to „Declaration‟.

k. After this no correction can be effected.
a. Clicking on to the Payment Section, details of fee charged under different category will be displayed with total amount to be paid.

b. Click on “GENERATE PAY-IN- SLIP” and take out the print copy of the challan and Log Out.

c. Go to the nearest State Bank of India Branch, deposit the amount as printed in the challan, Bank will give a Journal number on the copies of challan meant for Board & School.

d. Keep the school copy for future reference and send the Board‟s copy of the challan and copy of the Descriptive Rolls to the concerned zonal office of the Board by 09/11/2015 for verification and further course of action.

e. Re-visit the link by Log In and enter the Journal Number & “SUBMIT”

f. Genuineness of the journal number submitted by the Head of Institutions will be verified by the Board. In case it is found that fake journal number is submitted, criminal proceedings will be initiated against Head of the Institution.
