How To Check Base Line Survey (BLS) 2012 Status Report & Family ID Online
STEP 1: Click on following button to enter in the official website. The page will load as Format A3 - Swachh Bharat Mission Target v/s Achievement of BLS-2012 (Entry Status).
STEP 2: After page goes fully load, select your State Name from the drop down menu situated in left column. Wait for few seconds and then select your District Name (situated in the middle column). Now select your Block name and click on “View Report”. SEE IMAGE.
After clicking “View Report” the website will load all Gram Panchayat Lists with Entered Details (With & Without Toilet) BLS-2012 for your selected Block. In the table you can see the details of Entered Beneficiaries Numbers including BPL & APL for all Gram Panchayats.
STEP 3: Click on “Blue Colored Numbers” situated in 3rd column against your Gram Panchayat Name to get detailed status of all beneficiaries for villages which are under your Gram Panchayat. SEE BELOW IMAGE.
After clicking on that number, the page will load all beneficiaries list with details such as their Name, GP Name, Village Name, Habitation Name, Family ID, Father/Husband Name, Gender, Job Card/ EPIC/ Aadhaar/ MGNREGA Number/ Driving License/ BPL/ Antyodayo Card, etc. details, Category (BPL/APL), Caste Sub Category (General, SC, Small & Marginal Farmers, etc.) and the status for Whether Having Toilet Before Survey, Whether Covered After Survey and Status of Toilet Access.
STEP 4: Now search for the beneficiary name in the list. You can use the Search Box to search names. If the search box goes fail (Server error or any other error), manually check for names by browsing page by page list. SEE BELOW IMAGE.
In addition, you can download Complete Beneficiaries List in Excel (.xls) format by clicking “Download complete Beneficiary details” situated in right-top section of the web page. Also, you can download single page as Excel (.xls) & Word Format (.doc) by clicking Excel & Word Icon situated in right-top section of the web page. You can print the page by clicking CTRL + P on your Keyboard.
